POSITION TITLE: CRS Clinician

DEPARTMENT: Clinical Services

FLSA STATUS: Exempt


REPORTS TO: Lead Clinician – Level 3 – Community Response Services

GENERAL DESCRIPTION OF DUTIES

The CRS clinician is responsible for responding to crisis calls/referrals with a combination of focus of a community response specialist and clinician. This individual provides specialized crisis response services, assessments for mental health/substance use, and coordinating a plan of care for those encountered during a crisis call. Clinicians will de-escalate crises, educate/administer Narcan when needed, provide case management and linking participants to community resources and programming, transporting clients as necessary, collecting data for ongoing quality management and program development, coordinating with Police/Fire/EMS and mobile crisis teams if needed.

SPECIFIC DUTIES AND RESPONSIBILITIES

ESSENTIAL JOB FUNCTIONS

  • Respond to crisis calls and/or referrals
  • Provide brief immediate crisis intervention.
  • Perform outreach activities and prevention education to foster good community relations, educate about program services and assess needs in Hartford neighborhoods where there are high crime and utilization of 911 for crisis intervention.
  • Adhere to program protocols and practices as it relates to staff and client safety, operations, and program documentation.
  • Participate in regulatory audits.
  • Manage a caseload of program participants and provide low-threshold referrals and connect participants with medical care, substance use and mental health treatment, and other life-stabilizing services.
  • Ensure referrals and support services are made to outside programs for case management and medical intervention if applicable.
  • Refers participants to the agreed upon treatment program and participates in the initial treatment planning with the identified treatment provider.
  • Assess participants, develop, implement, and evaluate a comprehensive crisis plan with each participant maintaining regular contact until participants engage into treatment/community resource or declines need for further assistance. Refer to community resources as needed.
  • Provide weekly or as needed, office or community visits to program participants as applicable.
  • Maintain follow-up contact and documentation requirements for every program participant.
  • Provide on-going liaison, referral and advocacy for participants.
  • Correspond with community providers as it relates to programs contents area.
  • Maintains and disseminates listings of resources such as basic needs, recovery meetings, employment and housing assistance programming.
  • Engage high-risk, hard to reach people who struggle with mental health, drug use, or engage in sex work.
  • Identify potential recruitment locations. Engage in dialogue with participants and other community members to continually improve our services and ensure that CRT program operations efficiently and engage in outreach activities in the best locations at the best times.
  • Keep accurate, consistent, complete records and data on all program activities and/ or participants; provide program director with reports in a timely manner.
  • Prepare, set up and dismantle program sites and other program materials. Ensure the integrity of the sites, and maintain inventory of program supplies and bi-lingual educational materials
  • Complete vehicle daily maintenance logs which include oil, tires, gas, and structural checks. Document and report any incidents or accidents within 24 hours of occurrence. Ensure that vehicles are kept clean, properly stocked, and ready for service

ADDITIONAL JOB FUNCTIONS

  • Maintain compliance with complete yearly mandatory training including confidentiality and HIPAA compliance.
  • This position is considered as an essential employee and will be expected to work during inclement weather, holiday and based on business needs.
  • Flexibility to work in various settings including participation in outreach within Hartford, CT.
  • Perform other duties as assigned.

MINIMUM TRAINING EXPERIENCE AND MEDICAL CERTIFICATIONS

Education: Bachelors and/or Master’s degree in social work or related field from an accredited institution.

Minimum Years of Experience: Over three (3) years of experience working with marginalized populations.

Demonstrate skills: Proficient in databases, MS Office, written reports and maintenance of case notes. Strong organization, motivational, time management and interpersonal skills. Specialized training and background in crisis intervention, substance use and/or mental illness, domestic violence and/or any other applicable training related to field of work preferred. Experience working with participants from diverse ethnic and economic backgrounds with the ability to interact and communicate effectively.

Knowledge: Knowledge of human services, criminal Justice, court systems, homelessness, substance use, trauma, and mental health. Thorough knowledge of co-occurring disorders and of social services in Hartford with the demonstrated ability to work in collaboration with other service providers required.

Active Driver License in good standing required upon hire and throughout employment

Physical Ability: Tasks involve the ability to exert very moderate physical effort in light work, typically involving some combination of stooping, kneeling, crouching and crawling, and which may involve some lifting, carrying, pushing and/or pulling of objects and materials of moderate weight (12-20 pounds).

Sensory Requirements: All tasks require visual perception and discrimination and the ability to communicate daily in oral and written form.

Environmental Factors: Tasks are regularly performed with exposure to adverse environmental conditions, such as dirt, dust, pollen, odors, wetness, humidity, rain, fumes, temperature and noise extremes, machinery, vibrations, electric currents, traffic hazards, animals/wildlife, toxic/poisonous agents, violence, disease, or pathogenic substances.
