Job Overview

Community Physicians is seeking an experienced and compassionate APRN (FNP or Psych Certified) or Physician Assistant to join our growing team in a psychiatric setting. You will round at Skilled Nursing Facilities (SNFs), Assisted Living (AL), and Independent Living (IL) communities, working collaboratively with physicians to provide high-quality psychiatric care throughout the Northern Illinois region. This role offers autonomy, flexible hours, and the chance to build lasting relationships with patients and facility staff.

Responsibilities

• Conduct psychiatric evaluations and medication management
• Collaborate closely with supervising physicians
• Provide mental health services to geriatric and adult populations
• Maintain accurate clinical documentation
• Build professional relationships with facility staff and families
  
  

Qualifications

• Licensed APRN (FNP or Psych) or PA in the state of Illinois
• Experience in psychiatric care strongly preferred
• Strong communication, organization, and work ethic
• Ability to work independently and collaboratively
• Valid driver’s license and reliable transportation
