This is an exciting opportunity to join our expanding interdisciplinary team in the highly desirable Phoenix/Scottsdale area. You will help shape and grow outpatient palliative care services within a physician-led, innovative health system.

Key Responsibilities
- Core Faculty HonorHealth Palliative Care Fellowship Program
- Provide outpatient palliative care in clinic setting
- Manage complex symptoms (pain, dyspnea, fatigue, etc.)
- Lead goals-of-care discussions and advance care planning
- Collaborate with interdisciplinary care teams (APPs, social work, chaplaincy, nursing)
- Coordinate care with primary care and specialty providers
- Contribute to program expansion and innovation
- Optional teaching and mentorship opportunities
