Key Responsibilities
Medical Coordination & Support
- Schedule and coordinate medical, dental, and therapeutic appointments
- Transport individuals to and from appointments using agency vehicles
- Maintain accurate and up-to-date medical records, charts, and documentation
- Prepare and process medical paperwork including referrals, releases, care plans, and medication records (MARs)
- Coordinate medication management, including pickups, documentation, and communication of changes
- Monitor and address medical-related needs, including insurance coordination and issue resolution
Documentation & Compliance
- Ensure timely, accurate completion and distribution of all required documentation
- Maintain organized filing systems for medical records and medication documentation
- Generate reports and utilize internal databases for tracking and compliance
Team Collaboration
- Communicate effectively with nurses, providers, program staff, and pharmacies
- Support and assist team members as needed
- Maintain a professional, respectful, and service-oriented approach in all interactions
- Participate in training and ongoing development
What We're Looking For
- High school diploma or GED required
- Experience in human services or a medical setting preferred
- Valid Colorado driver's license with acceptable driving record
- Ability to pass a criminal background check
- Willingness to obtain Medication Administration, First Aid, and CPR certifications within 90 days (paid training provided)
- Strong organizational skills and attention to detail
- Ability to manage multiple priorities and work with minimal supervision
- Effective communication and problem-solving skills
Success In This Role Looks Like
- Keeping medical appointments, records, and medications highly organized and up to date
- Proactively identifying and resolving issues before they become problems
- Communicating clearly and consistently with all stakeholders
- Managing a high volume of tasks with accuracy and efficiency
Physical & Work Environment
- Regular walking, standing, and occasional lifting (up to 50 lbs independently, 100 lbs with assistance)
- Exposure to medical environments, including potential contact with infectious materials (training and precautions provided)
Supervision
This position reports directly to the Medical Supervisor and does not have supervisory responsibilities.
