Certified Occupational Therapy Assistant (COTA) – PRN

Community Nursing Services (CNS) | Home Health and Hospice | Utah

Position Summary

CNS is seeking a compassionate PRN Certified Occupational Therapy Assistant (COTA) to support Home Health and Hospice patients across CNS's service areas throughout Utah. Working under the supervision of a licensed Occupational Therapist, you'll carry out treatment plans that help patients regain independence in the activities that matter most to them — dressing, bathing, cooking, moving safely through their own home. Meaningful 1:1 care in the patient's own environment, with real autonomy and a mix of adult and some pediatric patients.

Compensation & Schedule

  • Pay: $45 per visit. really won't list it as per visit.
  • Mileage: Provided for qualified travel.
  • Classification: PRN, non-exempt hourly employee.
  • Schedule: Monday through Friday, with weekend work available if you want it.

What You'll Do

  • Provide occupational therapy services within the scope of your practice and licensure as a Certified Occupational Therapy Assistant
  • Carry out the plan of care established by the supervising Occupational Therapist — therapeutic exercise, ADL and self-care retraining, functional mobility, and cognitive or perceptual retraining
  • Instruct patients, families, and caregivers in home programs, energy conservation, joint protection, and safe use of adaptive equipment
  • Assess the home for safety concerns and recommend modifications that reduce fall risk and support independence
  • Report changes in patient status, response to treatment, or home situation to the supervising Occupational Therapist and care team promptly; participate in case conferences as requested
  • Support hospice patients with comfort-focused positioning, adaptive equipment, and caregiver training consistent with their goals of care
  • Document care accurately and on time, maintain patient dignity and privacy, and follow safety, infection control, and HIPAA guidelines

What You'll Bring

  • Associate degree from an ACOTE-accredited Occupational Therapy Assistant program
  • Current, unencumbered Utah license or certification as an Occupational Therapy Assistant, maintained throughout employment; NBCOT certification preferred
  • Home health, hospice, skilled nursing, or outpatient rehab experience preferred — new graduates with strong clinical fieldwork will be considered
  • Working knowledge of ADL and IADL retraining, adaptive equipment, and Medicare/Medicaid documentation standards for home health and hospice
  • Clear understanding of the OT/COTA supervisory relationship and the boundaries of the COTA scope of practice
  • Ability to work independently in diverse home settings, adapt treatment to the realities of a patient's home and available equipment, and know when to escalate to the supervising OT
  • Strong communication, organization, and time management; comfort with electronic documentation; and the empathy to handle emotionally difficult situations well

Other Requirements

  • Valid driver's license, current auto insurance, and reliable transportation for travel to patients in the communities your office supports
  • Current CPR certification, or obtained within 30 days of hire
  • Ability to pass a criminal background check and drug screening
  • Reliable phone and internet access for scheduling and documentation in the field, and willingness to transport therapy supplies and adaptive equipment between visits

Physical Requirements

  • Ability to lift, transfer, and reposition patients (up to 50 lbs frequently, more with assistance), and to guide and physically assist during transfers, gait, and functional mobility
  • Ability to sit, stand, walk, bend, kneel, squat, and navigate stairs and varied home environments
  • Manual dexterity to demonstrate fine motor tasks, fit adaptive equipment, and perform hands-on techniques
  • Ability to drive throughout your assigned CNS office area and surrounding communities
