We're looking for a licensed Physical Therapist to join our team and provide high-quality, one-on-one care to patients in the comfort of their own homes.
What You'll Do
- Conduct initial assessments and comprehensive care evaluations
- Develop and implement individualized treatment plans tailored to each patient
- Monitor and document patient progress, adjusting plans as needed
- Apply physical therapy modalities to treat patients experiencing pain, limited mobility, and endurance or stamina challenges
- Collaborate with a multidisciplinary care team to ensure the best outcomes
What We're Looking For
- Current Pennsylvania Physical Therapist license (required)
- 1 year of physical therapy experience preferred
- Strong clinical judgment and excellent communication skills
- Ability to work independently in a home-based care setting
