Certified Home Health Aide: Full-Time
Requirements:
- Completed a Certified Nursing Assistant (CNA) training program and have an active license
- Active CPR Certification
- 1 year of relevant experience as a CNA
- Compassion, integrity, and a commitment to providing excellent care
- The ability to handle situations with confidentiality and maturity
- The ability to read, write, and speak English
- Commitment and dependability
- Rotating weekend and holiday on-call responsibilities
Primary Job Responsibilities:
- Care for patients in their homes in conjunction with the rest of the clinical team.
- Follow a personalized care plan for each patient that is assigned by the case manager who is available to support you in your responsibilities.
- Assist with activities of daily living (ADLs), light meal preparation, companionship, respite when applicable.
- Promote a safe environment and assist with ambulation/transfers.
- Maintain communication with the interdisciplinary team and the patient’s caregivers.
Pay: $22.00 - $24.00 per hour
