Job Summary:

Registered Nurse (RN) must abide by the CMS Conflict of Interest Policy. Come to work (in person/onsite) as scheduled and consistently demonstrate dependability and punctuality. The incumbent must consistently work cooperatively with administration, nursing service personnel, physicians, community agencies, Persons served, families and consultants. Additionally, the incumbent is required to always present a professional appearance, accept assigned duties in cooperative manner from DON & QIDP, and assume accountability for data contained in the employee’ s handbook.

Job Description:

  • Coordinate Health Program for Residential Group Homes and Waiver Services.
  • Make daily Person visits and report status of nursing care to the Director of Nursing Service and QIDP/ Manager as assigned
  • Supervise Nursing Staff (LPN & TME). Adhere to Standards of Nursing Practice, Nursing service objectives and performance standards
  • Determine appropriate level of need based on Nursing assessment
  • Assess Persons care needs and develop and monitor Health Care Management Plans and Passports.
  • Provide Medical Documentation in a timely manner for all incidents and or investigations for Persons supported under care in a timely manner
  • Annual and quarterly Nursing Assessment
  • Participate as part of the Interdisciplinary Team
  • Compare physician orders with each Person’s plan of care to assure appropriate implementation and consistency
  • Complete Monthly Nursing Progress Notes and Acute Care Plans.
  • Update emergency medications periodically. (Minimum of 5 Day supply for each Person)
  • Observe Infection Control Policies and Procedures.
  • In-Service Training to Staff in personal Hygiene, family life and sex education, CPR/ First Aide, Adaptive Equipment, Universal Precaution and other areas as identified.
  • Analyze Quality Indicators reports, QA IMC, DDA reports to identify concerns and implement corrective action to improve Persons care.
  • Assist Physician whenever needed.
  • Serves on Committees Participate on Safety and Infection Control Committee and Human Rights Committee
  • Maintain confidentiality of Person and Group Home information.
  • Administer medication whenever needed.
  • Assess Person adaptive equipment and medical supply needs, and make recommendations to the Director of Nursing Service.
  • Consistently make accurate level of needs determination. Complete reports and assignments accurately and adhere to established time schedules.
  • Establish open lines of communication between all nursing personnel and consistently take follow-up action as necessary.
  • Check lab schedules and prepare the forms.
  • Prepare referral slip for those who need specialty appointments
  • Follow upon lab result, x-ray, mammogram, etc. Have Dr. review them and file it in the medical book.
  • Observe infection control procedures relating to nursing services.
  • Be responsible for the safety of Persons under his/her care or supervision.
  • Observe Persons’ Rights policies at all times.
  • Observe all Group Home safety policies and procedures.
  • Write monthly nursing notes for all of your Persons including weight and blood pressure.
  • Write readmission nursing note for all readmitted Persons after Hospitalization.
  • Do initial nursing assessments for new admissions.
  • Take temperature twice a day for those on antibiotics if ordered by Physicians.
  • Review new monthly MARS and sign and date.
  • Review Physicians order sheet, sign and date. Place in MD box.
  • 33.Follow up on discharge recommendations from E.R. and hospitalizations.
  • Attend Monthly Nursing meeting.
  • Meet with QIDP and Managers weekly. Check that medical appointments for follow-ups, labs, X-rays, etc. are done on time.
  • Check creams and lotions book for documentation.
  • Check first aid supply kits.
  • Give weekly reports to the DON.
  • In-service training for staff and person under care.
  • Provide physical exam, lab test, licensure and CPR (First Aid annually)
  • Abide by the CMS Conflict of Interest Policy; see attachment. 30 day notification when submitting resignation.
  • Come to work as scheduled and consistently demonstrate dependability and punctuality.
  • Consistently work cooperatively with administration, nursing service personnel, physicians, community agencies, Persons, families and consultants.
  • Come to work in a clean, neat and consistently present a professional appearance.
  • Accept assigned duties in cooperative manner from DON & QIDP.
  • Assume accountability for data contained in the employees’ handbook.
  • Perform other related duties as directed by his/her supervisor.(DON / QIDP)

Qualifications:
Valid District of Columbia RN License Minimum one (1) year of experience as an RN. Must be able to lift weights of at least sixty (50) pounds. Strong written and verbal communication in the English language. Excellent organization skills and attention to detail with the ability to prioritize and meet deadlines. Possess strong ability to think critically, make decisions, and maintain confidentiality. Ability to collaborate and work in a team environment. Candidates must provide proof of valid RN license in DC and CPR and First Aid certifications. All candidates will be required to pass a drug test and physical.

Job Type: Full-time

Pay: $83,200 per year
