Job Summary:

Licensed Practical Nurse (LPN) must abide by the CMS Conflict of Interest Policy. Come to work (in person/onsite) as scheduled and consistently demonstrate dependability and punctuality. The incumbent must consistently work cooperatively with administration, nursing service personnel, physicians, community agencies, Persons served, families and consultants. Additionally, the incumbent is required to always present a professional appearance, accept assigned duties in cooperative manner from DON & QIDP, and assume accountability for data contained in the employee’ s handbook. This position may require working some weekends.

Job Description:

  • Read nurse’s log daily.
  • Monitor Individuals daily for any change in health status.
  • Administer medication at designated times.
  • Daily documentation on MARS.
  • Check adaptive equipment weekly (helmets, walkers, gait belts, wheelchairs etc.)
  • Monitor Bowel Charts for daily documentation
  • Check lab schedules and prepare the forms
  • Prepare referral slips for those who need specialty appointments
  • Review any Incident Reports, Medical Service Sheet, and Health Concerns Forms (in the nurse’s box daily at each Home).
  • Communicate with the Primary Care Nurse, QIDP, and Residential Managers of any concerns.
  • Write weekly nursing note for all People/Persons including weight, blood pressure, pulse and temperature.
  • Sign In/Out when going to each Home.
Medication:
  • Review physician’s order sheet, note and transcribe orders.
  • Review MARS both for oral medications and creams and lotions.
  • Check the cycle meds against MARS and POS.
  • Order medication and new orders call using Omnicare Care.
  • Pick-up over-the-counter medications to be started the same day.
  • Update emergency medications periodically (make sure each Individual has a 5-day supply).
  • Review and take the cycle medications to the home before the end of the month, place in charts.
  • Review Medical Appointment Sheets for sedation.
  • Assist Physician as needed.
  • Perform other related duties as directed by his/her supervisor (DON/QIDP).

Qualifications:

Candidates must possess the following qualifications: Valid District of Columbia LPN License Minimum one (1) year of experience as an LPN. Must be able to lift weights of at least sixty (50) pounds. Strong written and verbal communication in the English language. Excellent organization skills and attention to detail with the ability to prioritize and meet deadlines. Possess strong ability to think critically, make decisions, and maintain confidentiality. Ability to collaborate and work in a team environment. Candidates must provide proof of valid LPN license in DC and CPR and First Aid certifications. All candidates will be required to pass a drug test and physical.

Job Type: Part-time (Minimum 32 hours per week)

Pay: $30 per hour
