Job Summary

Duties
- Provide assistance with daily living activities, including personal hygiene, meal preparation, and medication management.
- Monitor and document client progress and changes in behavior or health status.
- Implement individualized care plans tailored to each client's needs and preferences.
- Facilitate social interactions and community engagement for clients to enhance their social skills and independence.
- Maintain a safe and supportive environment by adhering to safety protocols and guidelines.
- Ensure compliance with HIPAA regulations regarding client privacy and confidentiality.
- Collaborate with healthcare professionals, family members, and other support staff to coordinate care effectively.
- Participate in training sessions to stay updated on best practices in caregiving, patient observation, and memory care.
Qualifications
- High school diploma or equivalent; additional education in social work or related fields is a plus.
- Previous experience in caregiving, senior care, hospice care, or nursing home settings preferred.
- Certification in CPR is desirable.
- Strong patient observation skills with the ability to monitor changes in clients' conditions effectively.
- Knowledge of memory care techniques and strategies for supporting individuals with cognitive impairments.
- Excellent communication skills, both verbal and written, to interact effectively with clients, families, and team members.
