Job Summary
The Family Empowerment Case Planner carries a caseload of up to 10 families, providing up to three months of crisis intervention, conflict mediation, and family stabilization services. This position works closely with families, schools, and community providers to de-escalate crises and build lasting skills for positive communication and self-advocacy.
Essential Duties and Responsibilities
- Carry Caseload of 10 families and provide up to 3-month intervention to families diverted from the family court to de-escalate their crisis and mediate their disputes.
- Participate in ongoing training in evidence-based models.
- Crisis intervention and family stabilization to establish positive communication and planning
- Collaborate with the Department of Education. Obtain school records, coach parents on ways to become an advocate for their child and arrange a visit to the school.
- Coach parents and children in effective communication.
- Facilitate self-advocacy in both parents and youth.
- Conduct a combination of home and office visits, tele-conference and when necessary, phone sessions
- Adhere to all training requirements mandated by ACS to maintain casework skills as well as skills to manage the data collection, reports and other requirements.
- Make appropriate, effective referrals to community-based service providers.
- Provide summary in progress notes in meetings with the other providers and collaterals.
- Document timely and accurate progress notes and client records obtained.
- Participate in case audits and case closure.
- Maintain case records and record keeping in compliance with ACS requirements/and all existing Federal, State and City laws. This includes CONNECTIONS and PROMIS systems.
Experience, Education and Qualifications
- Bachelor’s degree required and a minimum of 1 years’ experience in Child Welfare desired; bilingual Spanish a plus.
- Strong interpersonal and writing skills.
- Effective time management and flexibility are key
- Be available for emergencies and crises during evenings/weekends.
- Optimistic about change and have a “do what it takes” attitude.
- Must be able to work effectively with a team.
- Work schedule based on program needs and is subject to change where necessary.
Physical Requirements (Essential Functions)
This position requires the ability to move throughout the facility and perform tasks that may include:
Standing or sitting for extended periods
Bending, squatting, or reaching
Occasionally lifting or carrying up to 30 pounds
