Role Description The Client Medical Coordinator provides support and instruction to direct care staff to ensure that clients are properly supported. Coordinate client medical care with direct care staff, Program Support Coordinators and Client Financial Coordinators. Provide support to adults with developmental disabilities to live independently in the community. Support services are based upon individual needs, wants, and desires. Services are focused on maintaining health and safety while promoting independence and an increased quality of life through consistent instruction and support.
MINIMUM QUALIFICATIONS:
At least 18 years of age.
High school diploma or GED equivalent.
Proof of eligibility to work in the United States.
Possess a valid driver’s license.
Possess an insured and safe vehicle.
Must be able to read, write, speak, and understand English.
Ability to pass a DSHS background check.
Satisfactory motor vehicle report.
Possess strong knowledge of medical terminology and a basic understanding of medicine relevant to coordinating client care.
Possess excellent communication skills for interacting with clients, families, medical professionals, and internal teams.
Proficiency with basic computer applications and electronic health or client record systems.
Ability to work with minimal supervision.
Ability to work effectively under high stress/pressure situations.
Ability to work a flexible schedule, which may include early mornings, late evenings, and weekends.
Ability to initiate independent and appropriate judgement and decisions.
PHYSICAL QUALIFICATIONS:
Must be able to remain in a stationary position at least 50% of the time.
Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.
Ability to frequently operate a computer and other office productivity machinery, such as a copy machine or printer.
Ability to frequently communicate with employees, clients, providers, guardians, etc. through phone and/or email.
Ability to occasionally reach, bend or lift up to 20 pounds, possibly more with assistance.
Ability to frequently operate a motor vehicle.
Ability to perform CPR and First Aid, as needed.
PREFERRED QUALIFICATIONS:
Two years’ experience in direct delivery of support services and instructions to people with developmental disabilities, with a minimum of one year of experience in a tenant support-like program.
Previous experience in healthcare, social services, or medical coordination.
