What You’ll Do

As a Case Manager, you’ll serve as the primary advocate and service coordinator for individuals receiving support. Your responsibilities will include:

• Meeting regularly with individuals based on their Individual Plan of Service (IPOS)

• Coordinating services and monitoring progress toward personal goals

• Assisting with access to entitlements like Medicaid, Medicare, SSI, and more

• Supporting health and safety planning, including crisis resolution and discharge coordination

• Helping individuals set and achieve S.M.A.R.T. goals—logistically, financially, and emotionally

• Facilitating community inclusion, relationship-building, and self-advocacy

• Coordinating housing, transportation, income generation, and school transitions

• Collaborating with families, providers, and support networks to deliver creative, individualized services

• Documenting services, progress, and plans using agency systems and protocols

• Participating in trainings, workshops, and continuous quality improvement initiatives

What You Bring

• Bachelor’s degree in a human services field (required)

• Meets qualifications as a Qualified Intellectual Disabilities Professional (QIDP)

• Social work licensure (LLBSW, LBSW, LLMSW, LMSW) preferred but not required

• At least 1 year of experience supporting individuals with intellectual/developmental disabilities

• Strong communication, organization, and computer skills

• Commitment to person-centered planning, self-determination, and community integration

• Positive attitude, team spirit, and a drive to learn and grow

Travel Requirements

This is a dynamic field-based role with extensive scheduled and unscheduled travel. You’ll visit homes, schools, hospitals, and community locations to provide hands-on support.

• Must provide your own transportation

• Valid Michigan driver’s license, vehicle registration, and insurance required

• Alternate drivers must be pre-approved by Human Resources

Physical Demands

• Ability to walk, bend, stand, sit, lift up to 25 lbs., stretch/reach, hear, see, and drive

• Reasonable accommodations available in accordance with applicable laws
