Community Infusion Solutions (CIS) is seeking a results-driven Care Coordinator Manager to lead a high-performing team responsible for patient coordination, referral management, and operational support for outpatient infusion programs.

What You’ll Do
✅ Lead Daily Operations
- Oversee care coordination and care coordination support teams and workflows including referral intake, scheduling, and patient tracking
- Ensure timely processing of referrals and adherence to service expectations
- Monitor team productivity, queue management, and workload distribution
✅ Drive Quality & Compliance
- Conduct routine quality reviews to ensure accuracy, completeness, and compliance with client requirements
- Review complex or high-cost cases for documentation and guideline adherence
- Escalate non-compliant cases to leadership as needed
✅ Develop & Manage Your Team
- Lead, coach, and mentor Care Coordinators and Care Coordinator Support teams to meet performance and productivity goals
- Manage timecards, PTO requests, and attendance tracking
- Identify coaching opportunities and implement performance improvement plans
✅ Partner with Clients & Internal Teams
- Serve as a key point of contact for client communication and issue resolution
- Collaborate with clinical, billing, and leadership teams to ensure continuity of care
- Respond to client requests, audits, and escalations promptly
✅ Improve Processes
- Identify workflow inefficiencies and implement solutions to improve patient access and team performance
- Ensure consistency and accuracy in documentation across the team
What We’re Looking For
Preferred
- Clinical background (LVN, RN, or similar)
- Experience with prior authorizations, referrals, and payer requirements
- Knowledge of CPT and ICD coding

Compensation & Benefits
- Base Salary Range: $55,000-$65,000

Qualifications:
Required
- Bachelor’s or associate’s degree in healthcare, Nursing, Business, or related field
- 2–5 years of experience in care coordination, or healthcare operations
- 2 years of leadership or supervisory experience
- Strong understanding of healthcare workflows and patient coordination processes
- Proven leadership, organizational, and problem-solving skills
- Excellent communication and client facing skills
Salary : $55,000 - $65,000
