Expanding Our Care: Opening a Rural Health Clinic

Through Inclusion Care Solutions, we are launching a new Rural Health Clinic focused on providing accessible, community-centered medical care that empowers people to stay healthy, stay local, and stay connected. This clinic is the next natural step in our journey to provide holistic, wrap-around support that prevents isolation and promotes true community integration.


Role Description

This is a full-time, on-site position for a Nurse Practitioner located in Winchester, KY. To bring this vision to life, we are seeking a dedicated Nurse Practitioner (NP) who shares our passion for equity and empowerment. In this role, you will be much more than a medical provider; you will be the clinical anchor for an underserved area. By providing high-quality, local care, you will directly dismantle the healthcare barriers that hold people back. Responsibilities: 

  • Rural Family Medicine: Serve as the lead clinician for patients across their entire lifespan, managing chronic rural health issues (diabetes, hypertension, obesity) and providing acute care for injuries or illnesses. 
  • Specialized IDD Care: Act as a clinical advocate for the IDD population, navigating the comorbid conditions and polypharmacy. 
  • Behavioral Health Integration: Conduct evaluations to support the mental health diagnosis and provide ongoing medication management as required by providing support for anxiety, depression, and neurodivergent-specific needs. 
  • Autonomous Decision-Making: Exercise the broad scope of practice unique to rural settings under the supervision of the Medical Director, including performing office procedures, ordering/interpreting diagnostic tests, and managing care transitions (e.g., pediatric to adult medicine). 
  • Recovery & Wellness: Support those recovering from substance use disorders and the co-occurring conditions that may arise. 


Qualifications: 

Education & Training and/or Experience: 

  • Education: Graduate of an ARC-PA-accredited Physician Assistant program. 
  • Licensure: Current, unencumbered Kentucky Physician Assistant (PA-C) license. 
  • Clinical Experience: Minimum of 3–5 years of experience providing primary care.  Behavioral health services are strongly desired, specifically to individuals with intellectual and developmental disabilities (IDD). 
  • Resilience: Maintain professional composure during high-stress moments or patient crises. 
  • Field Adaptability: Ability to work in varying conditions, including exposure to common clinical/household environmental factors and moderate noise levels. 
  • Manual Assistance: Ability to regularly lift and/or move up to 50 pounds and occasionally up to 100 pounds
  • Physical Redirection: Physical flexibility and endurance to safely redirect or control individuals if they become physically aggressive. 
  • Environmental Tolerance: Willingness to work in settings that include potential exposure to body fluids and blood-borne pathogens. 

 

Language/Math/Reasoning Ability: 

  • Communication: Ability to read and comprehend instructions and write clear, professional daily progress notes, behavioral observations, and medical necessity letters. 
  • Synthesis: Ability to effectively present complex clinical information in one-on-one situations to participants, families, and non-clinical support staff. 
  • Logic & Diagnostic Reasoning: Ability to apply advanced reasoning to ensure physical ailments are not misattributed to a patient's disability or psychiatric condition  
  • Math: Ability to add and subtract two-digit numbers and perform basic operations involving units of American money.


Licenses, Police Check, Registration, and Certifications: 

  • Driver's License: Current valid Kentucky Driver's License and maintenance of proper vehicle insurance coverage. 
  • Certifications: Valid DEA registration and active KASPER account for monitoring controlled substances. 
  • Safety Training: Ability to obtain and maintain First Aid/CPR certification and CORE training from the College of Direct Support. 
  • Criminal Record Check: Must pass an Administrative Office of the Courts (AOC) check via the Kentucky Abuse Registry and Employment Screening (KARES) system. 
  • Central Registry Check: A cleared Child Abuse and Neglect (CAN) check or KARES screening for Kentucky-only history. 
  • Caregiver Misconduct Registry: Verification through the Caregiver Misconduct Registry or KARES for Kentucky-only records. 
  • Nurse Aide Registry: A formal check through the Nurse Aide Registry or KARES for Kentucky-only status.
