The Housing Facilitator serves as the first point of contact for access to CHN Housing. This position is responsible for assessing and screening all applicants from all referral sources for CHN housing. The Housing Facilitator will work within the community through CHN-engaged partnerships to assist target population individuals in their housing searches. The position will also guide individuals in the screening and qualification process, ensuring compliance and stakeholder requirements are met.

What you will do in the role:

• Applicant Assessment & Screening - Conducting housing assessments, reviewing applicant files, and addressing barriers to placement to connect individuals with appropriate CHN housing. Coordinating with referral systems and partner agencies, responding promptly to inquiries, and performing timely assessments in the community to ensure effective housing access and support.
• Administrative & Clerical Management - Maintaining accurate files and records to meet compliance requirements and respond to audits, ensuring all documentation supports subsidy and housing eligibility. Monitoring waitlists, preparing residents for move-in by verifying documentation, and collecting program data, generating service utilization reports, and submitting grant reports as required.
• Collaboration with Funders, CHN Partners, & Lead Agencies - Partnering with ADAMH, behavioral health organizations, and lead agencies to identify consumers and coordinate access to rental subsidies and permanent housing. Participating in external meetings and maintaining positive relationships with case workers and agency representatives.

What you bring to the role:

• Bachelor’s degree in social work, Psychology, Human Services, or related field.
• 2 years’ experience in social services work
• Valid driver’s license and auto insurance
• Excellent written and verbal communication skills required with the ability to communicate across all levels both internally and externally.
• Proficient in Microsoft Office Suite Products and ability to learn and adapt to changing technology utilized in role.
