Within our Diversion Services is the Forensic Projects for Assistance in Transition from Homelessness, better known as our FPATH team. The FPATH Program is a voluntary intensive outreach program that works with adults with severe persistent behavioral health issues. The Washington State Health Care Authority contracts with CHMHA to find and offer services to people who have had multiple requests for competency evaluations in the last two years. Our F-PATH team provides free companion-based case management to help people connect to resources with the goal of connecting people in our program to long term behavioral health treatment to break their cycle of incarcerations.
Our Diversion Services here at CHMHA has a fantastic opening for an F-PATH Case Manager.
Our F-PATH Case Manager will work as part of the FPATH team to provide comprehensive community supports for adults in transitional community-based supported housing and to meet established FPATH Program goals. Residential Case Managers will work within a team model to provide needed services, linkage, and coordination. Coordinating with other providers is an integral part of the job. The FPATH team utilizes a harm-reduction, trauma-informed, and client centered approach.

RESPONSIBILITIES
Develop rapport with individuals with extreme barriers to engaging in services
Assist in client development of necessary skills to live independently such as meal prep, laundry, hygiene, etc
Assist with transporting clients to appointments
Provide immediate crisis intervention and resolution as needed
Complete risk assessments and help people build de-escalation skills/strategies
Coordinating with other relevant professionals (behavioral health, legal, medical, etc)
Support the F-PATH Emergency Housing intake/interview process
Support clients with following up with legal expectations with goal of reducing recidivism
Engaging with clients under the influence of drugs and/or alcohol and clients in severe poverty who may have poor hygiene
Utilizing a Harm Reduction approach with demonstrated passion for serving individuals with co-occurring mental health and substance use disorders at various stages of change
Advocacy for clients and assisting clients in learning to advocate for themselves
Timely completion of required documentation to program standards
Adhere to HIPAA/Confidentiality standards- Assist in staffing the drop-in/resource center
Other duties as assigned
QUALIFICATIONS
Bachelor’s degree in social services related field
Have legible handwriting
Valid driver’s license and clean driving record for use of agency vehicles
Experience working with individuals with severe and persistent mental illness, individuals with chronic substance use issues, and individuals involved in the criminal justice system is preferred
Strong diagnostic & assessment skills along with expertise with Co-Occurring disorders
Knowledge of or background in working in the forensic/competency system
Ability to work a non-traditional schedule including some holidays
Basic understanding of the criminal justice system
Sensitivity to multi-cultural populations and issues
Must be eligible and able to attain Washington State Agency Affiliated Counselor License
Ability to perform duties outlined in description within reasonable accommodation
