Full-Time Caring Crew

Community Horizons is searching for a morning and an evening caregiver for a man who suffers from MS. The caregiver in this position needs to show dedication and reliability due to his care needs. You will be helping this gentleman with any personal care, along with any supportive home care.

Job Tasks:

• Assist with transferring from bed to wheelchair using a pivot transfer or a sit-to-stand lift.
• Assist with personal hygiene tasks (showering, dressing, hair care, etc.)
• Light housekeeping (sweeping, toileting, laundry, linen changes)
• Meal preparation
• Errands as needed (grocery shopping or pharmacy pick up).
• Assist with AFOs.
• Assist with any daily range of motion
• Assist with incontinence care.

Shifts available:

- 6:00 AM – 2:00 PM

- 3:00 PM – 11:00 PM

Every other weekend is required

Pay: 

- $13.00- 18.32 PM

A breakdown of the wage can be discussed

Requirements:

• Ability to pass a Wisconsin Caregiver background check.
• Valid Driver's license and reliable vehicle.

Who is Community Horizons?

Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program.

Self-direction allows participants to make their own choices—selecting who provides their care and how those services are delivered.

While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork.
