Job Overview
Community Horizons is looking to connect with compassionate and dedicated Self-Directed Caregivers to provide personalized in-home and community-based care to individuals with diverse needs. This role offers flexibility and the opportunity to make a meaningful difference in community members lives by supporting their independence and well-being. Ideal candidates are experienced in various aspects of patient care, including assisting with activities of daily living (ADLs), medication management, and specialized care for individuals with developmental disabilities, dementia, or autism.
Duties
- Assist clients with activities of daily living such as bathing, dressing, grooming, and toileting
- Support residents with mobility using equipment like Hoyer lifts and assist with transfers
- Prepare nutritious meals tailored to dietary needs and preferences
- Administer medications accurately following prescribed care plans and medication schedules
- Provide companionship and emotional support to enhance quality of life
- Assist with Supportive home care tasks such as cleaning and maintaining cleanliness or home.
- Support clients during outings or community activities as needed
Qualifications
- Prior experience in home & community care, assisted living, nursing homes, or hospice care preferred
- Knowledge of patient observation, vital signs monitoring, medication administration, and ADL assistance
- Certification in CPR and First Aid required; additional certifications such as HIPAA compliance are a plus
- Experience working with individuals with developmental disabilities, autism, or dementia highly desirable
- Ability to lift heavy loads safely (e.g., Hoyer lifts) and perform physically demanding tasks when necessary
- Strong communication skills and compassionate approach to caregiving
- Valid driver’s license preferred for transportation responsibilities
