Schedule: M, W, F (8a-9:30/10a)
Wage: $16.50
Tasks:
· Bathing Assistance
· Floor Care (sweep, mop and vacuum)
· Cleaning the bathroom tub after use.
· Laundry
· Light Meal Prep
Ideal Candidate:
· Female
· Must wear mask
· Caregiving Experience
· Task Focused

· Animal Lover (home has 3 cats)
· Must be able to work in a smokey environment
· Must be able to pass a background check
Who is Community Horizons?
Community Horizons provides support to individuals and families enrolled in self-directed care programs across Wisconsin, including services through Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and the Children’s Long-Term Support (CLTS) program. Self-direction allows participants to make their own choices, selecting who provides their care and how those services are delivered.
While Community Horizons offers guidance and mentorship, we are not the employer of record. Participants and families make all hiring decisions, with a third-party fiscal agent handling payroll and employment paperwork. Our mission is to help individuals of all ages navigate self-directed care with connection, coordination, and compassion
