The position of Team Leader is one of great responsibility, developing a team of professionals to assist in the care of the people we serve.  This position requires working in all areas of responsibilities; medical, financial, clerical work, maintaining staff, and maintaining the home of the individual in all aspects (shopping, cleaning, etc.). It is necessary for the person in this position to be flexible, understanding and an excellent role model for all the Team Members.  The Team Leader must be able to cover all shifts as needed.

 

Listed below is a responsibility list (not inclusive) of a Team Leader.

 

Medical

Be aware of and schedule medical, dental, psychiatric, and optical appointments 

Transport individuals to doctor appointments

Maintain the prescriptions ordered by physician

Maintain all necessary medical records:

  1. Medications sheets
  2. B. M. Sheets
  3. Record Consumers’ weight monthly & height every 6 months in Medical Record Book
  4. Health care chronological
  5. Standing medical orders
  6. Medical appointment record
  7. Dental appointment record
  8. Psychiatric appointment record
  9. Standing Missed Med. Order
  10. Meet with community nurse at least quarterly
  11. Meet with community dietitian at least quarterly

 

Individual 

  1. Be aware of all personal preferences and follow them in meeting the individual’s needs
  2. Assist in all areas which are required by the individual
  3. Assist in maintaining a safe and clean environment for the individual 
  4. Assist in maintaining appropriate clothing for the individual
  5. Assist in proper hygiene of the individual (bathing, hair care, nail care, etc.)
  6. Assist in appropriate social and recreational activities for the individual

 

Fire and Safety

  1. Develop and maintain fire safety plan 
  2. Develop emergency evacuation plans
  3. Conduct and document fire drills monthly
  4. Conduct and document tornado drills March through October

  

Programmatic

  1. Monitor daily living skills programs
  2. Make suggestions to supports coordinator for ADL skills enhancement
  3. Tally and submit data for daily living skills programs
  4. Keep data sheets updated 
  5. Meet with supports coordinator once per month
  6. Meet with OT/PT quarterly
  7. Attend all meetings with outside support staff

 

Financial 

  1. Daily check of household and individual funds
  2. Weekly balance of funds sheets
  3. Monthly balance of checking/savings accounts
  4. Set up and maintain budget systems for each individual
  5. Weekly banking for each individual
  6. Organize and maintain receipts.  Sign receipt, circle date and amounts
  7. Set up and prepare for monthly meeting with accounting department at office to go over funds

 

Day Program

  1. Transport to and from day program
  2. Maintain daily communication with day programs
  3. Meet with day program personnel once per month

 

Household 

  1. Assure house is clean 
  2. Shop for groceries and necessary household items
  3. Maintain menus which follow recommended dietary guidelines
  4. Maintain grocery list
  5. Maintain cleaning check sheet for staff
  6. Maintain household maintenance (make necessary repairs or contacts)
  7. Will complete & supervise Van Maintenance Schedule

 

Team Members

  1. Train and assist all Team Members in meeting the requirements of their positions, use forms 100, 107, 176, 396, and 510 as your tools.
  2. Assure all Team Members have all necessary forms and training requirements for CMH and Community Homes to include CPR/First Aid and Recipient Rights training
  3. Maintain two-week schedule within budgeted guidelines
  4. Ensure that Team Members are using ADP to accurately represent their hours worked
  5. Prepare mileage reimbursement forms for Team Members
  6. Prepare activity reimbursement forms for Team Members
  7. Meet with each team member weekly
  8. Work all different shifts at least one time monthly (i.e. day, afternoon, midnight, weekends)
  9. Conduct Team Member meeting once per month
  10. Conduct Performance Evaluation for each Team Member annually
  11. Use Corrective Action/Performance Improvement processes to address Team Member performance concerns
  12. Reward Team Members for completing job responsibilities 

 

 

Community Homes

  1. Submit two week schedules at least 8 days in advance, no later than every Payday by 12pm 
  2. Ensure weekly ADP reports and schedules match
  3. Submit mileage forms on the last day of each month
  4. Submit activity reimbursement on the last day of each month 
  5. Submit monthly program data sheets to office on the last day of each month
  6. Inform office of all meetings (In writing, two weeks or more prior to meeting)
  7. Attend monthly Team Leader meeting
  8. Attend necessary training sessions
  9. Make sure all shifts are covered at all times
  10. Have a working mobile phone at all times
  11. Return all phone calls/text messages/emails in a timely manner
  12. Assure all necessary forms are at home sites for Team Members use
  13. Will perform other duties as requested by Community Homes, Inc., supervisors and administration
