Job Summary:
The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN) and in accordance with physician orders, agency policies, state regulations, and Medicare Conditions of Participation. The LPN promotes patient independence while ensuring safe, compassionate, and high-quality care.
Essential Duties and Responsibilities:
- Provide skilled nursing care as ordered by the physician and directed by the supervising RN.
- Monitor and document patient condition, vital signs, and response to treatment.
- Administer prescribed medications and treatments within the LPN scope of practice.
- Perform wound care, catheter care, ostomy care, and other skilled nursing procedures.
- Observe for changes in patient condition and promptly report findings to the RN and physician as appropriate.
- Reinforce patient and caregiver education regarding medications, disease management, safety, and home care instructions.
- Maintain accurate, complete, and timely clinical documentation in the agency's electronic medical record.
- Follow infection prevention and control policies.
- Participate in case conferences and communicate effectively with the interdisciplinary team.
- Maintain patient confidentiality in accordance with HIPAA regulations.
- Comply with all federal, state, Medicare, and agency policies and procedures.
Pay: $50.00 per hour
