Community Home Health Care Services is looking for compassionate and caring Licensed Practical Nurses and Registered Nurses to join our team of home health providers. If you love one-on-one patient care, and a flexible schedule, home health may be the perfect fit for you. Our home health team enjoy full autonomy and continuum of care for each of their home bound patients.

Full-time, part-time, and PRN positions are available. We cover home visits to homebound patients in West Michigan along the lake shore. We need nurses to cover home health visits in Muskegon County, Oceana County, and Ottawa County. What area are you interested in covering?

Requirements:
- Graduate of nursing school and currently holding a RN or LPN license.
- 1 year of previous nursing experience, preferred
- Previous home health care experience, preferred
- Ability to communicate effectively and work as a part of an interdisciplinary team
- Strong ability to use critical thinking and deduction when performing job tasks and evaluating patient needs
- Ability to consistently connect with patients one-on-one
- Responsible, compassionate, and caring demeanor

Job Responsibilities:
- Providing services in accordance with the patient’s Plan of Care
- Performing initial evaluations, building Plan of Care, and regularly reevaluating patient’s nursing needs
- Initiating appropriate preventative and rehabilitative nursing procedures
- Preparing clinical and progress notes for each patient visit via EMR
- Preparing summaries of care conferences on patients in a timely manner
- Informing Physician and Director of Clinical services of changes to patient’s condition and/or needs as appropriate
- Counseling the patient and family/significant others in meeting nursing and related needs
- Supervising and teaching other nursing personnel and CNAs/HHAs
- Completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant change in condition

Compensation is based on experience and is negotiable.
