About the Role

Community Home Care Services is looking for caring, self-directed RNs and LPNs to provide skilled nursing visits to patients in their homes across Starke and Marshall Counties. This is a good fit for nurses who want flexible, per-visit work, enjoy one-on-one patient care, and like working independently in the community.

Key Responsibilities

Registered Nurse (RN)

• Conduct start of care, resumption of care, recertification, and discharge visits, including OASIS assessments
• Develop and update individualized plans of care in coordination with the patient’s physician
• Supervise and direct LPNs and home health aides according to agency policy and regulations
• Provide skilled interventions including wound care, IV therapy, medication management, and disease management teaching

Licensed Practical Nurse (LPN)

• Perform routine skilled nursing visits under the direction of an RN and the established plan of care
• Provide wound care, medication administration and reconciliation, vital sign monitoring, and other skilled tasks within LPN scope of practice
• Report changes in patient condition promptly to the supervising RN

All Nurses

• Educate patients and caregivers on disease processes, medications, safety, and self-care
• Complete accurate, timely visit documentation in the agency’s EMR
• Communicate with physicians, the care team, and families to coordinate care
• Follow infection control, safety, and HIPAA standards in the home setting

Qualifications

Required

• Active Indiana RN or LPN license (or a multistate compact license valid in Indiana)
• Current CPR/BLS certification
• Valid driver’s license, reliable transportation, and current auto insurance
• Ability to work independently and manage time effectively across multiple home visits

Preferred

• 1 year of nursing experience; home health experience a plus
• RNs: experience with OASIS assessments
• Experience with electronic documentation on a tablet or laptop
• Familiarity with the Starke and Marshall County area

Compensation

• Per-visit pay, negotiable and based on experience
• Mileage reimbursement for travel between visits

Expected hours: 5.0 – 29.0 per week
