JOB DESCRIPTION 


The Project Manager for Grants & Contracts provides organization-wide support across a diverse portfolio of Medi-Cal managed care agreements, government contracts, and grant-funded initiatives. This role offers broad, flexible project management capacity to ensure programs meet contractual obligations, compliance requirements, reporting timelines, and organizational goals. The Project Manager serves as a cross-functional partner who can support any stage of the grants and contracts lifecycle—from project planning and implementation to reporting, monitoring, and closeout—based on evolving organizational needs. 


The Project Manager will contribute to a wide range of activities spanning project management, contract and grant compliance, reporting, budget coordination, and stakeholder communication. While specific assignments may shift based on funding requirements and internal priorities, responsibilities include supporting: 

  • Coordination of grant- and contract-funded projects from initiation through implementation and closeout. 
  • Monitoring of deliverables, timelines, reporting requirements, and quality expectations across multiple funder types. 
  • Cross-department collaboration with Program Operations, Finance, Data/Evaluation, HR, IT, and Compliance. 
  • Preparation and submission of narrative, performance, data, and fiscal reports.
  • Development and maintenance of tools and systems to track deadlines, documentation, and progress. 
  • Participation in proposal development, renewals, and contract modifications. 
  • Communication with external partners, including health plans, government agencies, and philanthropic funders 

PRIMARY RELATIONSHIPS 

The position reports to the Executive Director, Strategy and Impact and works collaboratively as part of Community HealthWorks’ Management team. Within the organization, the position has primary working relationships with the Executive Cabinet, Directors, Project Managers, Community Health Workers, Housing Leads, and service providers. Outside the agency, the position coordinates with project subcontractors, key stakeholders, government agencies, and community partners, and, when applicable, the organization’s Grant Writer/Grants Consultant to support proposal development, reporting alignment, and funder communication. 


SUPERVISORY RESPONSIBILITIES 

The position does not supervise staff at this time but may supervise future Grants & Contracts staff as the department grows. 


ESSENTIAL DUTIES AND RESPONSIBILITIES 

Project Coordination & Implementation 

  • Provide day-to-day project support across Medi-Cal managed care contracts, government agreements, and philanthropic grants. 
  • Develop adaptable work plans, timelines, and communication structures that can be applied across multiple funding streams. 
  • Coordinate internal stakeholders to ensure alignment on deliverables, staffing needs, timelines, and progress. 
  • Facilitate ongoing project check-ins, identify risks, and support timely corrective action. 

Grants & Contracts Compliance 

  • Track and monitor a wide range of deliverables—operational, programmatic, fiscal, quality, and data-related—across different funders. 
  • Support compliance with Medi-Cal, county, state, federal, and philanthropic requirements, in coordination with Compliance function. 
  • Maintain audit-ready documentation, including SOWs, MOUs, budget files, amendments, data use agreements, and reporting history. 
  • Support subrecipient or partner monitoring when applicable. 

Budget, Fiscal & Invoicing Coordination 

  • Collaborate with Finance on budget monitoring, projections, and allowable cost considerations. 
  • Support documentation for deliverable-based payments, cost-reimbursement claims, and invoice packets. 
  • Track budget modifications, spending patterns, and alignment between program activities and funder guidelines. 

Data, Quality & Reporting 

  • Coordinate with Data/Evaluation teams to compile encounter data, outcomes, metrics, and compliance documentation required for Medi-Cal and other funders. 
  • Assist with the preparation of narrative and data-driven reports for leadership and funders. 
  • Contribute to quality improvement and performance monitoring activities by tracking trends and assisting in continuous improvement processes. 

Funder & Stakeholder Engagement 

  • Serve as a point of contact for health plans, county/state agencies, and private funders as needed. 
  • Support preparation for site visits, monitoring reviews, and operational check-ins. 
  • Prepare materials, presentations, and summaries for external and internal stakeholders. 

Proposal, Renewal & Pre-Award Support 

  • Contribute to proposal components such as timelines, work plans, staffing models, and required attachments. 
  • Support internal routing processes for new awards, amendments, and renewals. 
  • Provide organizational support during periods of growth, contraction, or shifting funding requirements.

Requirements

Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience: Bachelor’s degree in public administration, public health, business, social services, or related field required. 3 years of experience leading projects, administering grant, managing contracts and/or government-funded program operations experience in nonprofit, philanthropic, or mission-driven organizations. Experience with Medi-Cal managed care, Medicaid programs, or publicly funded health or human services is strongly preferred. Ability to manage competing deadlines and adapt to changing priorities. Demonstrated ability to work across multiple funding streams with varying requirements. A combination of skills, experience (including lived experience) may be considered as substitute for education and/or experience. 

  

Certificates, Licenses, Registrations: A current CA driver's license and proof of auto insurance are required. Must have reliable transportation and be able to travel within Sacramento County and surrounding counties.  


Other Qualifications: Must complete a criminal background check. Knowledge of the Sacramento health care delivery system, housing and social service resources. 


Physical and Emotional Demands:  This position requires the ability to manage multiple deadlines, shift priorities, and work with diverse internal and external stakeholders. The role involves periods of high information volume, detailed analysis, and independent decision-making. Standard office-related physical demands apply, including extended periods of computer use.

The employee is, occasionally required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms; climb or balance; stoop, kneel, and frequently talk or hear. The employee must occasionally lift and/or move up to 10-15 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
