Come join our growing Healthcare Provider Team! Community HealthNet, Inc.(CHN), a Federally Qualified Health center Organization (FQHC), with 5 health centers in Lake County, Indiana, only 30 minutes from downtown Chicago, is looking to employ a full-time Optometrist to join our integrated patient care team.

JOB SUMMARY

In this role, the Optometrist will ensure excellence in patient care by conducting eye exams, addressing issues of eye disease and/or injury, and educating patients regarding their eye care options. The Optometrist will treat patients of all ages with compassion and work collaboratively with the rest of our staff to meet all patient needs. Successful candidates will have a proven record of excellent patient care and a valid Indiana state license to practice optometry.

ESSENTIAL JOB FUNCTIONS

  • Examine the eyes of CHN patients, of all ages, for evidence of disease, injury or other problems impacting vision.
  • Perform various tests to determine visual acuity and perception and to diagnose diseases and other abnormalities such as glaucoma and color blindness.
  • Analyze results from all tests conducted and share results with patients.
  • Listen to and address patient’s questions and concerns.
  • Educate patients regarding their eye care and vision correction options.
  • Prescribes eyeglasses, contact lenses, and other vision aids or therapeutic procedures to correct or conserve vision.
  • Prescribe legend drugs when needed.
  • Utilize all necessary equipment properly, ensuring it is in good working order.
  • Coordinate with other CHN staff, as needed, to meet any/all CHN patients’ needs.
  • Record all patient tests, prescriptions, and treatments in the patient's EPIC/OCHIN EHR chart.
  • Consult with and refer CHN patients to Ophthalmologist or other health care practitioner(s), if additional medical treatment is determined necessary.
  • Participate in staff development and training.

Education and Other Requirements:

  • Graduate as an optometrist from a program which meets the fulfillment of educational requirements for licensure as an Optometrist in the State of Indiana.
  • Minimum of two years experience working as a practicing Optometrist
  • Solid working knowledge of all ophthalmic equipment needed to conduct examinations
  • Experience prescribing and fitting corrective lenses and contact lenses
  • Current medical knowledge of testing and treatments for eye injuries and diseases
  • Continued commitment to remaining up to date on the latest research and techniques
  • Positive attitude and team-oriented approach to patient care
  • Compassionate and professional chair-side manner
  • Able to maintain a clean and safe workspace
  • Bilingual in English and Spanish is a plus

Licensure:

  • Holds valid Indiana Optometrist Licensing
  • Holds valid Indiana Optometric Legend Drug Certificate
  • Meets Credentialing guidelines of CHN

PHYSICAL DEMANDS AND ENVIRONMENTAL CONDITIONS

  • Work usually performed in a clinic office environment
  • The ability to drive between two CHN clinics to see CHN Optometry patients
  • Prolonged periods of walking, sitting and standing
  • Frequent viewing of a computer monitor
  • Specific vision abilities required include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus
  • Frequent hearing and speaking required
  • Frequently required to reach with hands and arms
  • Frequent use of hands and fingers for repetitive motion
  • Frequent use of hands to touch, handle, grip and/or feel
  • Constant interaction with staff/clients/patients
  • Subject to exposure of bloodborne and airborne pathogens

MACHINES, TOOLS, EQUIPMENT AND WORK AIDS

Clinical diagnostic equipment regularly used in an Optometric setting, Computer, EPIC/OCHIN Electronic Health Record Software, fax machine, copier, and multi-line telephone system.
