In this role, you will be responsible for providing psychosocial-emotional support and services to patients and families. At any given time, you will be working with 150-200 patients, meaning that you will always have the opportunity to learn and encounter a variety of patients. Our clinic functions as a multi-disciplinary team and is comprised of 7 social workers, 4 RN's, and a medical office team. This means you will always be encountering a variety of care plans and pyschosocial needs. As a member of our team, you will serve as a point of contact in order to assist patients and family in understanding, coping and adapting to the person, emotion and bio-psychosocial issues.
Education
o Master's Degree in Social Work (MSW) or Master's Degree in Marriage and Family Therapy, Counseling, Psychology, Clinical Mental Health or related field with an emphasis in Marriage and Family Therapy required

Education
o Master's Degree in Social Work (MSW) or Master's Degree in Marriage and Family Therapy, Counseling, Psychology, Clinical Mental Health or related field with an emphasis in Marriage and Family Therapy required
Experience
- 1 year of clinical, case management, social work or related experience required
- Case management experience in healthcare/social services agency preferred
- Experience in an inpatient or outpatient care setting preferred
- BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
- CCSP - State of California Children's Services Paneling preferred within 180 days of hire or transfer into position
