Assertive Community Treatment Team (ACTT) serves adults 18 years of age and older. ACTT provides intensive community-based treatment, support, and rehabilitation to individuals who have a serious persistent mental illness (SPMI) or a dual diagnosis of SPMI and a Substance Use Disorder and be determined as requiring a high level of service needs.
Duties & Responsibilities
Experience: Personal experience with recovery and a commitment to supporting others on their journey is highly valued in this role. Minimum of 2 years’ experience with associate’s degree and (1) year experience with a bachelor’s degree working in a setting with individuals with substance use problems.
Licensure/Certification/Registration: C.A.C. or C.A.D.C. preferred.

Assertive Community Treatment Team (ACTT) serves adults 18 years of age and older. ACTT provides intensive community-based treatment, support, and rehabilitation to individuals who have a serious persistent mental illness (SPMI) or a dual diagnosis of SPMI and a Substance Use Disorder and be determined as requiring a high level of service needs.
Duties & Responsibilities
- Provide psychoeducation counseling and support to individuals and groups with substance use disorders, utilizing both professional training and personal recovery experience.
- Educate clients and their families about behavioral health and substance use disorders, treatment options, and coping strategies.
- Coordinate and manage client care with community providers and other referral agencies to ensure comprehensive and continuous treatment in collaboration with the client’s assigned clinician/therapist and clinical team.
- Meet all requirements of program contracts, including productivity, caseload management, reporting, and timeliness of client contact.
- Perform tox screens, including observed urine screens, oral swabs, and/or hair testing.
- Attend training sessions conducted by CHR staff or external consultants as requested or required.
- Actively participate as a full member of the treatment team, attending all staff meetings and team meetings.
- Perform other duties related to the position as needed.
- Must have knowledge of substance use disorders.
- Must have excellent written and verbal communication skills
- Must be computer literate.

COMPENSATION*:
$20.9685/hour minimum and up commensurate upon experience
- Actual rates are determined at the time of offer and are based on relevant experience, as well as, adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)
