In this exciting role as a Per Diem Case Management Specialist at Community Health Resources, you will have the chance to make a real difference in the lives of our clients. Our residential program, Hendee Road Home, provides a safe and supportive environment for men with severe mental illness to transition back into the community.
Program Overview
We focus on empowering our clients to increase their independence and self-efficacy by teaching essential life skills, such as symptom management, coping mechanisms, medication management, healthy relationships, personal safety, financial literacy, substance use education, recovery support linkages, and sober activities. Our team also provides crisis intervention to minimize hospitalizations and emergency room visits.
This position requires a strong background in human services, excellent communication skills, and the ability to maintain accurate records using our EMR system. You will work collaboratively with a multidisciplinary team to develop individualized treatment plans that cater to each client's unique needs.
