About the Role
This position includes a sign-on bonus, payable according to company policy and contingent upon successful completion of required employment conditions.
Responsibilities
- Conduct and document patient health histories
- Assist providers in delivering high-quality, patient-centered care
- Manage patient flow, including triage and follow-up care
- Coordinate referrals to specialists and community services
- Provide health education to patients and caregivers
- Collect lab specimens (e.g., blood, urine, sputum)
- Support infection control, safety standards, and supply use
- Occasionally assist with front office duties (scheduling, discharge, etc.)
- Float between Community Health Net locations as needed
Qualifications
- Valid Pennsylvania LPN license & current CPR certification
- High school diploma or GED
Required Skills
- Strong assessment, organizational, and communication skills
- Knowledge of medical terminology and basic insurance types
- Ability to use computers and electronic health record systems
