The OBAT Nurse will ensure that all data is collected and entered into the appropriate systems, and that follow up
with patients to coordinate appointments is completed and documented. The position works in conjunction with the
Nurse Manager and CNO. The Nurse demonstrates excellent decision-making skills, problem solving, and attention
to detail. The position is responsible for highly sensitive information, which requires a clear understanding of medical
and behavioral health confidentiality.

Minimum Qualifications
Supervision Exercised: Program Assigned Medical Assistant
Environmental Working Conditions: Normal medical office environment of a busy Community Health Center
with patients to coordinate appointments is completed and documented. The position works in conjunction with the
Nurse Manager and CNO. The Nurse demonstrates excellent decision-making skills, problem solving, and attention

and behavioral health confidentiality.
Essential Duties And Major Responsibilities
- Organization & coordination of SUD schedules, distribution of patients across sites and provider time
- Be the patient contact and point person for accurate information for patients and providers regarding
- Patient contact link for patient counseling, as well as patient contact link for primary care.
- Provide support during screening, intake, induction, and stabilization & maintenance phases of patient
- Participate in dual diagnosis group and peer driven groups.
- Conduct nurse visits for initial assessment and follow up appointments.
- Required to attend mandatory program meetings and training.
- Complete case management initial assessment to determine case management needs.
- Develop individualized case management plan in coordination with the member and provider to reach timely
- Tracking, follow-up and reporting of all diagnostic services, diagnostic test results, referrals for diagnostic
- Communication with providers, patients, and the patient’s families when appropriate
- Plan, coordinate, monitor, evaluate and document progress toward meeting goals and objectives on the
- Attend any mandated continuing education/training.
- Maintains the treatment database with health information for those patients enrolled in the program and
- Demonstrates understanding and commitment to the mission of the health center.
- Demonstrates understanding and commitment to the established CHC Values and Standards
- Works independently on special nonrecurring and ongoing projects as needed. Perform related duties
Minimum Qualifications
- Valid Massachusetts nursing license (RN or LPN)
- 1-2 years’ experience working as a RN/LPN
- Strong understanding of substance use disorders
- Organized
- Caring and compassionate
