FUNCTION: Provides quality professional medical services to an especially diverse population with special needs in a primary care setting and limited by scope of practice
DUTIES:

Provides care to patients:

Obtains and documents medical history and physical exam.

Orders labs and procedures.
-
Makes referrals appropriately.
-
Manages medical emergencies.
-
Triages phone calls.
-
Evaluates efficacy of treatment.
-
Documents accurately and appropriately, including phone calls.
-
Formulates assessment and plan.
-
Understands and recommends appropriate community resources.
-
Administers injections and medications.
-
-
Advocates for patients.
-
Collaborates with designated physician(s).
-
Able to function independently.
-
Able to work flexible hours.
-
On call coverage optional depending upon level of training and functioning.
-
Participates in at least 2/3 of staff conferences.
-
Participates in training.
-
Advises about health and prevention.

Provides patient education
-
Participates in QA activities.
-
Participates in data interpretation and integration.
SKILLS:

Needs good patient relation skills.

Needs good teamwork skills.
QUALIFICATIONS:

State RN license
-
Completion of an approved Nurse Practitioner program
-
MSN unless grandfathered.
-
2 years experience minimum providing primary care.
-
Prescriptive Authority.
-
Board Certification.
-
Maintain BLS certification.
