What are the responsibilities and job description for the Registered Nurse - Team Lead position at Community Health Center?
Our Nursing team leads are responsible for supervising the clinic
operations and the coordination of the workflow of medical services for
a primary care team in delivering the highest quality patient care.
Responsibilities will include: Providing leadership to nurses, medical
assistants, and clinical support staff to accomplish organizational and
team goals for quality patient care as well as patient and staff
satisfaction. Scheduling appropriate staffing level coverage, with
responsibility for time and attendance management. Completing
performance evaluations and ongoing coaching and mentoring with team.
Conducting comprehensive patient assessments, managing superior plans of
care for the patient/family/care team. Assisting team in provision and
coordination of patient care, including care coordination and follow up,
panel coordination and management, medication reconciliation, barrier
assessments, phone triage and patient and family education. Participates
in QI activities with team and QI committee to consistently improve
outcomes and patient health. Promoting a continuous learning environment
for new and existing staff. Qualifications: Bachelor of Science in
Nursing, with 2 years experience in a leadership or supervisory role,
ideally in a primary care, outpatient setting. Mission driven, with
ability to thrive in a fast-paced team-based environment.
