What are the responsibilities and job description for the Clinical Admin position at Community Health Center?
DESCRIPTION The Clinical Admin Will Provide Administrative Support To
primary care teams in facilitating patient care and promoting positive
health outcomes. DUTIES WILL INCLUDE: Patient Care Coordination in
triaging calls and My Chart Messages, routing to appropriate team
members and closing communication loop with patients as needed.
Supporting Quality Measures, ensuring patients are current with all
Health Maintenace Goals and follow up visits, ensuring access to care.
Bridging barriers to care, including arranging for transportation,
translation services, coordination with outside agencies and completing
paperwork required. Documenting and updating computer systems (EPIC,
Sorian) To Order And Coordinate Patient Services QUALIFICATIONS
Candidates will need a working knowledge of medical terminology and have
2 years supporting a patient care team. Strong organizational and
computer skills are required, with demonstrated ability to thrive in a
collaborative, fast -paced environment!
