Position Overview
The Advanced Practice Provider is responsible for providing comprehensive, patient-centered outpatient medical care to individuals and families in a Family Medicine setting. Working within CHA's Patient-Centered Medical Home (PCMH) and Team-Based Care model, the APP evaluates, diagnoses, treats, and manages a broad range of acute and chronic health conditions across the lifespan, consistent with the provider's education, training, certification, and scope of practice.
The APP collaborates with an interdisciplinary team to promote prevention, wellness, access to care, and positive health outcomes. Depending on the health center location and operational needs, the position may also include providing continuity of care in inpatient and/or residential settings.
