Role Description The Population Health Coordinator is a full-time, on-site role based in Reno, NV. This position supports population health initiatives by identifying care gaps, coordinating outreach, and following up with patients to improve preventive care, chronic disease management, and quality metrics. Daily responsibilities include reviewing patient lists and reports, contacting patients by phone or electronic messaging, scheduling and confirming appointments, and documenting interactions in the electronic health record. The coordinator collaborates closely with clinical and care management teams, helping to organize education sessions, track outcomes, and support evidence-based protocols. The role also involves assisting with data collection for quality improvement projects and connecting patients to available resources and wraparound services.

- Clear and professional Communication skills, both verbal and written, to interact with patients and multidisciplinary teams.
- Working knowledge of Medical Terminology to accurately understand and relay clinical information.
- Experience with Training or patient education, including explaining care plans and health resources.
- Proficiency with electronic health records and basic data entry; ability to manage patient lists and reports.
- Attention to detail, organizational skills, and the ability to prioritize multiple tasks in a fast-paced care environment.
