Our services include access to telehealth, medical specialists, pharmacies, therapy, peer support, medical supplies, homecare, and relevant programs like HCBS, OPWDD, and CFTSS. We also provide transportation assistance, Meals on Wheels, care management, and day programs for both children and adults. Role Description

This is a full-time, on-site Coordinator role based in Brooklyn, NY. The Coordinator will oversee the effective coordination of Medicaid-approved services and community programs for clients, ensuring seamless service delivery. Day-to-day responsibilities include liaising with service providers, scheduling appointments, managing client documentation, conducting follow-ups, and assisting clients in navigating healthcare and community resources. Additionally, the Coordinator will monitor service quality and compliance while supporting clients to access the resources they need to thrive.

Qualifications

• Strong organizational and coordination skills to manage appointments, schedules, and service plans
• Effective communication and interpersonal skills to liaise with providers, clients, and internal teams
• Proficiency in documentation, reporting, and maintaining client records
• Knowledge of Medicaid-approved services, including telehealth, HCBS, OPWDD, and transportation assistance
• Problem-solving skills and the ability to offer support in a fast-paced environment
• Familiarity with community health programs and resources in Brooklyn, NY
• Experience in healthcare, social work, or community outreach is a plus
• Bilingual (Spanish preferred, but not required)
