Role Description
This is a full-time, on-site Coordinator role based in Brooklyn, NY. The Coordinator will oversee the effective coordination of Medicaid-approved services and community programs for clients, ensuring seamless service delivery. Day-to-day responsibilities include liaising with service providers, scheduling appointments, managing client documentation, conducting follow-ups, and assisting clients in navigating healthcare and community resources. Additionally, the Coordinator will monitor service quality and compliance while supporting clients to access the resources they need to thrive.
Qualifications
- Strong organizational and coordination skills to manage appointments, schedules, and service plans
- Effective communication and interpersonal skills to liaise with providers, clients, and internal teams
- Proficiency in documentation, reporting, and maintaining client records
- Knowledge of Medicaid-approved services, including telehealth, HCBS, OPWDD, and transportation assistance
- Problem-solving skills and the ability to offer support in a fast-paced environment
- Familiarity with community health programs and resources in Brooklyn, NY
- Experience in healthcare, social work, or community outreach is a plus
- Bilingual (Spanish preferred, but not required)
