What are the responsibilities and job description for the Elementary Student Assistance Program Liaison position at Community Guidance Center?
Complete appropriate documentation relevant to indirect case management services for Elemenatry Student Assistance Program (eSAP) cases and incoming calls. Maintain case notes, service plans, and relevant paperwork requirements to insure completeethical, uninterrupted clinical care for consumers exercising social responsibility. Maintain all documentation to fiscal standards, maintain complete files and clinical documentation.
Conduct appropriate consultation with the treatment team regarding clinical care and policies/procedures utilizing social learning. Attend regular supervision with the immediate supervisor and openly communicate all issues which impact quality of clinical care. Conduct external consultation with other community providers, schools and agencies as indicated regarding client care.
Attend department meetings and/or staffing conferences as assigned. Maintain appropriate professional continuing education requirements. Stay appraised of & follow all current agency policies and procedures. Comply with quality assurance guidelines. Maintain awareness of all Federal, State and Managed Care laws, policies, regulations and procedures regarding clinical care, Best Practices Guidelines, and appropriate fiscal requirements related to services provided.
Attend Elementary Student Assistance Program (eSAP) CORE team meetings, minimum of two meetings per month at each school. Provide consultation to CORE teams, teachers, guidance counselors, and school personnel on a routine as needed basis. Serve as a key liaison between the CGC and the school system, facilitating linkage to behavioral health services for consumers and educate the school system regarding available services through social learning.
Provide intakes, Mental Health and/or Suicide Assessments to students referred by the SAP teams, other educational personnel, and as assigned by Outpatient Services Director on an as needed basis in the school or office setting. Therapy modalities include individual, group, family and collateral psychotherapy utilizing commonly accepted theories and techniques of behavioral, emotional and cognitive interventions and the S.E.L.F model (Safety, Emotions, Loss and Future). Prepare clinical documentation and coordinate with consumers, families and school personnel.
Provide courtesy sessions when necessary to provide support for the student in the event of a time lapse between a MH or Suicide Assessment and initiation of opening at the Guidance Center. Sessions provided in the school setting.
Crisis Intervention in the school setting. This would include assisting in crisis situations at the schools by providing counseling for students in the aftermath of a crisis upon request by the school exercising emotional intelligence.
Handle incoming requests for services at the Community Guidance Center:
Collect needed information from the referral source
Determine what services are needed
Facilitate appropriate and timely referral to provide these service,
Complete the required documentation of the intervention
Prepare Releases of Information as indicated
Provide crisis intervention as needed
Schedule appointments when appropriate.
