What are the responsibilities and job description for the Targeted Case Manager position at Community Growth Behavioral Health Inc?
The targeted case manager shall provide case management services which shall include assisting clients in gaining access to needed financial and insurance benefits, employment, medical, social, education, assessment of functional abilities and needs, and other services. These support services include working with the client and his/her natural support system to develop and implement the service plan. It also includes follow-up to determine the status of services, and the effectiveness of activities related to the successful implementation of the service plan toward enhancing the recipient’s inclusion in the community.
JOB RESPONSIBILITIES
A. Case Assessment
B. Developing, Implementing and Reviewing the Case Plan
a. This will include working with a client (and any family involved) to address
issues related to the services provided. Services where the family is involved must
be clearly directed to meeting the identified needs of the client.
C. Linking and facilitating with appropriate services and resources identified in the service
plan.
D. Advocating for the acquisition of services and resources necessary to implement the
service plan.
E. Coordinating the delivery of services as specified in the service plan, with the help of the
client, his/her family and his/her natural support system.
F. Monitoring service delivery to evaluate client’s progress.
G. Documenting targeted case management activities and other clinical paperwork within
the specified timeframe.
H. Arranging for and coordinating after care services upon discharge from a
residential/inpatient facility when discharge planning is not covered by the family’s per
diem.
I. Knowledge of the Medicaid manual and all updated revisions.
J. Ongoing chart audits to ensure compliance and quality assurance of services provided.
K. Knowledge of DSM V.
L. Proficiency in utilizing community resources as supportive services.

M. Serving as a liaison for the client, family, referral source and other social service
agencies.
JOB REQUIREMENTS
Graduation from an accredited university with a minimum of a Bachelor’s Degree in Psychology,
Social Work, Marriage and Family Therapy, Mental Health Counseling or Human Service-
related field with at least 1 year (Recommended) experience working with the specific
population they are applying to work with (i.e. children with serious emotional disturbances or
adults with serious mental illness) and:
1. Must be trained in all company required trainings.
2. Must complete a company approved mental health targeted case management training
within 3 months of initially supervising case managers.
3. Must have knowledge of, and must comply with, state and federal statutes, rules and
policies that affect the target population.
4. Must have knowledge of available resources for the specific population that they are
working with.
5. Must have knowledge of the principles and practices of the discipline.
6. Demonstrates proficiency in verbal and written skills.
7. Ability to work with diverse groups and individuals.
8. Maintains current annual training requirements (see “case management personnel and
training criteria policy”).
9. Must successfully complete all criminal background screens annually.
