Position Summary

The Manager of Placement Coordination is responsible for leading and overseeing the Family Consultant team to ensure high-quality case coordination, compliance, communication, and support services for children and foster families served by Community Connections. This role partners closely with the Manager of Foster Homes & Intakes and DFACS to support placement activities for medically fragile children in foster care.

This position focuses on team leadership, case oversight, placement coordination support, compliance monitoring, and operational accountability. While the Manager of Foster Homes & Intakes owns placement matching and foster home readiness decisions, this role serves as a key partner in reviewing child needs, supporting placement coordination efforts, and ensuring Family Consultants effectively manage ongoing case responsibilities.

Key Responsibilities

Team Leadership & Oversight

  • Lead, supervise, and support the Family Consultant team and Family Consultant Supervisors.
  • Monitor staff performance, productivity, case management practices, and service delivery standards.
  • Provide coaching, accountability, and professional development to ensure staff effectiveness and consistency.
  • Support employee engagement and foster a collaborative, mission-driven culture.

Placement Coordination Support

  • Partner with the Manager of Foster Homes & Intakes regarding child placement needs and case coordination activities.
  • Review child intake information and provide input regarding placement considerations, child needs, and support requirements.
  • Collaborate with internal teams to support safe, stable, and appropriate placements for medically fragile children.
  • Assist with escalated case situations and placement-related concerns as needed.

Case Management & Compliance

  • Ensure Family Consultants maintain accurate, timely, and compliant documentation in accordance with DFACS requirements, RCCL licensing standards, and agency policies.
  • Monitor compliance across assigned caseloads, including case notes, service plans, communication requirements, and documentation deadlines.
  • Monitor compliance with contractual placement obligations
  • Ensure staff are trained on compliance standards and updates
  • Partner with Director of Foster Homes and Placement Operations to address compliance risks
  • Implement corrective actions when compliance gaps are identified
  • Maintain readiness for internal or external audits related to placement practices

DFACS & Stakeholder Partnership

  • Develop and maintain positive working relationships with DFCS Leadership and other licensing bodies.
  • Ensure consistent communication and follow-through related to child and family needs.
  • Participate in staffing meetings, case consultations, and operational discussions.

Training & Staff Development

  • Lead onboarding and training initiatives for Family Consultants.
  • Provide ongoing education related to trauma-informed care, medically fragile populations, documentation standards, and compliance expectations.
  • Identify process improvement opportunities and implement best practices across the team.

Operational Collaboration

  • Partner closely with the Manager of Foster Homes & Intakes to ensure alignment across placement coordination, foster home readiness, and child services.
  • Contribute to operational planning, workflow improvements, and service delivery enhancements.
  • Track and report on operational metrics, team performance, and compliance trends.

Qualifications

  • Bachelor’s degree in Social Work, Human Services, Psychology, Healthcare Administration, or a related field required. Master’s degree preferred.
  • Minimum of 5 years of experience in child welfare, foster care, social services, or related human services environments.
  • Previous supervisory or team leadership experience required.
  • Experience working with DFACS and foster care systems strongly preferred.
  • Experience supporting medically fragile children or complex care populations preferred.
  • Strong understanding of child welfare compliance requirements, case management practices, and trauma-informed care.
  • Excellent leadership, organizational, communication, and relationship management skills.
  • Ability to manage multiple priorities in a fast-paced, highly regulated environment.

Pay: $65,000.00 - $75,000.00 per year
