What are the responsibilities and job description for the Community Based Day Support Professional position at COMMUNITY CONNECTIONS INC?
Community Connections Inc. is in search of dedicated Community Based Day Support Professionals to join our Community Based Day Support team in South Yarmouth, MA. In this role you will support individuals with disabilities by creating a pathway to employment by providing career exploration and skill building activities. You will partner with individuals and businesses throughout Fall River and surrounding communities and aid Community Connections in their mission to provide optimal independence and enriching services to people with disabilities. We are looking for someone with a commitment to compassion, mentoring others and serving the community.
What You'll Do as a Community Based Day Support Professional:
- Develop and expand opportunities for community integration to address the pre-employment, social, and recreational interests of the individuals we serve.
- Conduct assessments of the individuals’ interests, skills and abilities to identify learning styles, social skills and available supports.
- Promote the development of skill acquisition and exponential learning through community exploration.
- Maintain relationships with community businesses and resources to secure opportunities for individuals we serve.
- Support individuals in becoming active, engaged members of their community by facilitating volunteer, leisure, and employment opportunities.
- Document individuals' progress toward goal objectives and keep up-to-date case records.
What You'll Need as a Community Based Day Support Professional:
- High School Diploma or General Education Degree (GED)
- Bachelor's Degree in human service or business-related field preferred
- 1 year's experience and/or training working with individuals with disabilities
- Valid driver's license, access to a motor vehicle, and satisfactory driving record
- Ability to work a flexible schedule
- Excellent written and verbal communication and interpersonal skills
- Demonstrated ability to work independently
- A proven ability to work independently and a proficiency in computer and word processing skills
