Individual Support Specialist
Community Connections, Inc. (CCI) is seeking a compassionate, organized, and motivated Individual Support Specialist to join our team. In this role, you will serve as the primary support for individuals on your caseload — coordinating services, developing Individual Support Plans (ISPs), and empowering individuals to live meaningful, independent lives.
This position involves regular travel to individuals’ homes and community locations to provide direct support and services.

What You’ll Do as an Individual Support Specialist:
- Coordinate and oversee all services for individuals on your caseload, including developing and implementing Individual Support Plans (ISPs)
- Travel to individuals’ homes and community settings to provide direct support and monitor services
- Maintain accurate case records, documentation, billing records, and required reports (daily, semi-annual, and annual)
- Provide direct care, role-modeling, and skills training aligned with each individual’s goals
- Support health, safety, hygiene, medication awareness, and overall well-being
- Promote community integration, social engagement, and meaningful activities
What You’ll Need as an Individual Support Specialist:
- High School Diploma or GED required
- Bachelor’s degree in Human Services or related field preferred
- Minimum one year of experience working with individuals with disabilities
- Valid driver’s license and reliable transportation required
- Ability to write routine reports and maintain accurate documentation
