JOB SUMMARY

With general supervision of the Program Director, is responsible for facilitation of home-based early intervention services (for children ages birth-three) in partnership with families and other team members. All Community Connections employees are expected to contribute to the professional reputation of the agency through understanding and promoting the organization’s mission and core values in all aspects of services.

DUTIES & ESSENTIAL JOB FUNCTIONS

Provides program support and administrative assistance to other program staff including processing referrals, creating/maintaining customer files in compliance with Part C and Medicaid regulations, managing information in electronic database systems, and maintains effective tracking system of enrolled children’s timelines and evaluations through closure of services;

Performs vision, hearing and developmental screenings associated with child find activities;

Plans and implements the development of the Individualized Family Service Plan and supports the Speech Language Pathologist in the developmental evaluation which may include note taking and recording, along with other activities;

Assists families in the identification of their concerns, priorities, and community resources available to the families;

Supports families and/or other primary caregivers to implement activities outlined in the Individualized Family Service Plan;

Completes necessary documentation within required timeframe including treatment notes, progress reports, and exit summaries.

KNOWLEDGE, SKILLS AND ABILITIES

Knowledge of infant, toddler, and child development;

Knowledge (broad) of mental illness and/or intellectual disabilities, physical disabilities, medical conditions, and/or treatment techniques;

Knowledge of computer applications related to the work, including but not limited to database, spreadsheets and word processing programs;

Skilled in and ability to records management principles and practices

Ability to professionally interact and communicate positively with diverse populations including families, team members, supervisors, and the public;

Ability to be friendly and outgoing, with a notably positive attitude;

Ability to interpret and implement company policies, procedures and systems, using initiative and independent judgment;

Ability to utilize flexibility, initiative and leadership, organize own work, coordinate projects, set priorities, meet deadlines and following up on assignments with minimal direction.

REQUIRED QUALIFICATIONS

Bachelor’s Degree in a related field (early childhood special education, early childhood education, K-8 elementary education) with State licensure (Speech Language Pathologist Assistant);

Must complete the Part C Credential process within first 6 months of employment.

Must be able to pass a criminal background check under agency guidelines and state regulations.

PREFERRED

Experience working with families and children under three.

REQUIRED LICENSURE/CERTIFICATION

Must obtain an Alaska Driver License, have access to vehicle to conduct agency business and ensure vehicle has liability insurance coverage.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT

This position requires the ability to perform general office duties such as typing, using office equipment and minimal lifting. Work is performed in office, families’ homes and community settings. Requires traveling to outlying communities in small planes or ferries and may include evening visits.

Pay: DOE
