Community Connection Healthcare, LLC is looking for a RN for Stoughton, Sharon, Attleboro and surrounding areas. Positions are part-time/ per diem and CCH will work with staff to build a flexible schedule.

Community Connection Healthcare and MassHealth AFC and GAFC service provider, seeks qualified registered nurse (RN) for non-skilled agency work (ADLs). Flexible schedule, intake assessments, and rotating visits.
- MassHealth AFC and GAFC programs knowledge
- Physician Summary Form (PSF) experience, PCP form experience
- Provide on-site MDS.
- Review and responsible for POC changes, requests, and updates as needed with patients change in significant health.
- Current RN License (and CPR); strong familiarity with behavioral health, addiction, and non-skilled healthcare
- Technology skills, ability to use apps, fax, scan, use Alora software or similar healthcare software programs, driver's license, phone, and own computer.
- Maintain confidentiality in all aspects of the job.
- Bilingual preferred: English and Spanish
Rate from $40-$45 based on visit type.
Requirements Include:
- RN certified
- CPR Certification required
- Updated TB skin test and Physical
- Background check required
- Driver's License/State ID required
- Travel, must have own transportation, flexibility (up to 7 days a week)
- Mature, responsible, attendance, and punctuality
- High standards of honesty, and integrity
- Must be able to work independently with little or no supervision
Pay: $40.00 - $45.00 per hour
