Title: Bilingual (English/Spanish) Social Worker
Reports to: Case Management Coordinator
Classification: Non-Exempt
Summary:
The Bilingual Social Worker (English/Spanish) will identify and engage individuals and families on the
South Shore of Long Island, assess their needs, connect them to appropriate services, and ensure
coordinated, culturally responsive care.
Essential Functions:
- Model and promote the organization’s mission, values, and guiding principles through
professional conduct, and adhere to all policies and procedures.
- Provide culturally responsive, client-centered case management and support services.
- Conduct comprehensive assessments, develop care plans, and evaluate ongoing client needs.
- Coordinate services by making referrals, facilitating linkages with community providers, and
conducting follow-up to ensure successful connections.
- Assist clients in accessing services, benefits, and entitlements through information, referral, and
short-term support.
- Advocate for clients, particularly in navigating systems with language or cultural barriers.
- Conduct outreach, build relationships with community stakeholders, and connect clients to
internal programs and external resources.
- Maintain accurate documentation, records, and program data to support quality service delivery
and evaluation.
- Provide crisis intervention and follow-up services as needed.
- Perform additional duties as assigned to support team and organizational goals.
Physical Demands and Work Environment:
- Employee must be able to travel to various off-site locations for outreach events, meetings, or
community engagement activities as needed, however the work will largely be performed in the
office.
Supervisory Responsibilities:
This position has no supervisory responsibilities.
Position Type/Expected Hours of Work
This is a full-time position, 40 hours per week. Days and hours are generally within Monday through
Friday, 9:00am-5:00pm, with occasional late evening hours.
Required Education and Expectations
- MSW degree required.
- Case management experience and knowledge of the local community preferred.
- Experience working with families.
Knowledge and Skills:
- Excellent interpersonal and communication skills required.
- Demonstrated competency in English written, verbal, and computational skills to present and
document records in accordance with program standards.
- Demonstrated competency in Spanish written and verbal skills.
