Community Support Worker (CSP)
Job Description
The Community Support Worker (CSP) is responsible for delivering person-centered, rehabilitative, and supportive services to individuals with behavioral health needs. The CSP Worker provides outreach, care coordination, health and wellness coaching, and linkage to community resources. This role requires strong communication skills, the ability to work independently, and a commitment to empowering clients to meet their treatment goals.
Key Responsibilities
Provide community-based outreach and support to clients with mental health and/or substance use challenges.
- Assist clients with developing coping strategies, life skills, and wellness plans.
- Support clients in maintaining housing stability, employment, education, and daily living activities.
- Provide crisis intervention, safety planning, and stabilization services as needed.
- Conduct culturally responsive and trauma-informed services.
Care Coordination
- Collaborate with interdisciplinary teams including therapists, case managers, primary care providers, hospitals, and state agencies.
- Assist clients in navigating community resources such as housing, benefits (MassHealth, SSI/SSDI), food security, and transportation.
- Coordinate referrals to behavioral health and medical providers.
- Advocate for client needs within various systems.
Preferred Qualifications
- Bi-lingual/bi-cultural Spanish
- Experience in substance abuse prevention, co-occurring disorder recovery and/or mental health field
- Associate degree or higher with degree in health and human services, behavioral health
- Experience working with immigrants, retirees, and Medicaid populations
- Knowledge of local health disparities and resources to address gaps and disparities
- Requirements and procedures for care coordination, data management, and Electronic Health Records
- Experience in conducting presentations on health and wellness and self-management of health conditions to individuals and groups
Required Qualifications
- Demonstrated knowledge of local population demographics, assets, and needs
- Demonstrated knowledge of Behavioral health.
- Effective communication in English And Spanish both orally and in writing.
- Demonstrated skills in organization and time management
- Pass a Background check
- Good driving record, reliable transportation, insurance up to date
- Preferred
- Experience working with diverse populations, including immigrants, refugees, and individuals experiencing homelessness.
- Multilingual abilities (e.g., Spanish)
- Familiarity with MassHealth behavioral health services and community resources.
Job Type: Full-time
Pay: $20.00 - $30.00 per hour
