Agency is now hiring MH/IDD Paraprofessionals to work with 3-5 years old.

SUMMARY:  Provide support to individuals in the MH/DD/SA population with severe and persistent mental illness.  


 Provide coaching and assistance with Instrumental Activities of Daily Living (IADLs) including but not limited to preparing meals, budgeting/managing money, shopping for household necessities, using the telephone to make appointments and link with natural supports, housecleaning, laundry, accessing transportation, transporting the individual to access the community, medication management, supervision, and cuing.

 

 ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: Other duties may be assigned.


1.       Assist with identifying interventions to address barriers that impede the development of skills necessary for independent functioning in the home/community.

  1. Provide input into person-centered plan, modifications, and treatment teams, as needed
  2. Coordinate activities within the established person-centered plan;
  3. Assist with referral linkage to other services and community resources
  4. Provide direct intervention, arrange, coordinate, and monitor services on behalf of the person’s supported

6.       One-on-one intervention with individuals receiving services to develop independent skills, interpersonal, and coping skills

7.       Assist person receiving services with development of skills for self-management and monitoring of symptoms.

  1. Minimize the negative effects of psychiatric symptoms or substance dependence that interfere with the person supported daily living and personal development
  2. Support the person in the development of various skill building activities and clinical interventions specific to the person’s goals and mental health diagnosis
  3. Complete documentation of services including a daily full service note that includes the name of the person supported, Medicaid identification number, record number, date of service, purpose of contact, describes the interventions, includes the time spent performing the interventions, effectiveness of the intervention, the signature and credentials of the staff providing the service.
  4. Work closely with Qualified Professional, other Associate/Para-professionals, and clinical/professional staff to maintain communication, provide feedback, and standardize procedures
  5. Facilitate relationships and serve as a link between the agency, parents, guardians, natural supports, other service providers, and the community
  6. Ensure that all documentation is accurate and reflective of services and completed on time as defined by the agency’s policy and state guidelines
  7. Attend supervision meetings, complete supervision plan, and complete tasks as assigned by Supervisor
