- Conduct thorough patient assessments, including vital signs, medical history, and physiology evaluations to determine appropriate care strategies.
- Collaborate with multidisciplinary teams to develop and implement individualized discharge plans that optimize recovery and post-acute care.
- Manage utilization review processes by evaluating diagnostic evaluations, medical records, and treatment plans to ensure efficient resource use.
- Coordinate intake procedures for inpatient, outpatient, clinic, and emergency room cases while maintaining compliance with HIPAA regulations.
- Facilitate communication between patients, families, healthcare providers, and managed care organizations to ensure continuity of care.
- Oversee case management activities across various specialties such as ICU, PICU (Pediatric Intensive Care Unit), Geriatrics, Behavioral Health, Infectious Disease Care, and more.
- Maintain accurate documentation of patient interactions, medical records, and discharge summaries while adhering to hospital policies and medical terminology standards. Experience
- Proven experience in hospital medicine or acute care settings with a focus on medical-surgical units or emergency departments.
- Critical care experience within Level I or Level II trauma centers is highly desirable.
- Background in ICU, OR (Operating Room), Geriatrics, Pediatrics (PICU), or post-acute outpatient clinics enhances your ability to excel in this role.
- Familiarity with diagnostic evaluation processes, inpatient/outpatient workflows, and utilization management practices is essential.
- Strong knowledge of physiology principles, medical terminology, and patient assessment techniques supports effective case management.
