Registered Nurse, Part-Time, Day Shift
Community Care is hiring a Registered Nurse for our Pace Program. This rewarding opportunity is perfect for someone who enjoys team collaboration, community nursing case management, being part of a mission driven organization and who shares our mission to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This is a part-time opportunity at our St Francis Convent in Milwaukee and the Siena Convent in Racine.
Registered Nurse Job Responsibilities:
- Manage caseload of Family Care Program members
- Complete comprehensive clinical assessments
- Working collaboratively to develop, member centered care plans, provide on-going care coordination, member education, quality assurance and monitoring of services.
- Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Registered Nurse Job Requirements:
- Education: Associates degree in Nursing Required, Bachelors Degree preferred.

Licensure: Wisconsin Licensed Registered Nurse

Licensure: WI Drivers License

Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity
Preferred 2 years of work in community nursing
Registered Nurse Job Schedule:
- Monday - Friday, 28 hours per week
- Flexible work schedule during our normal business hours of 8:00AM - 4:30PM
