Nurse Practitioner, Full-Time, Day Shift
Community Care is hiring a Nurse Practitioner for our Pace Program. This is a full-time opportunity at our Racine/Kenosha location.
***$10,000 sign on bonus***
Nurse Practitioner Job Responsibilities:
- Documents and updates health history and findings, physical exam, functional status evaluations, diagnostic test and lab evaluations, mental health status and function evaluations, preventative care, and health reminders.
- Within their official scope and practice, delegates appropriate tasks to interdisciplinary team staff, and provides leadership and guidance. Acts as a resource to staff to ensure appropriate care and follow through and provides care planning assistance.
- Educates members and families regarding health maintenance and acute and chronic illnesses.
- Travel, Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
Nurse Practitioner Job Requirements:
- Education: Masters degree in Nursing required.
- Licensure: Wisconsin Registered Nurse/RN license required. Family, Geriatric, or Adult NP certification is required. DEA registration required. Advance Nurse Prescriber required. NPI and CPR & AED required
- Experience: minimum of 1 year recent clinical nursing experience with an at risk or elderly population.
Nurse Practitioner Job Schedule:
- Monday - Friday, 40 hours per week
- 8:00AM - 4:30PM
- On-Call responsibilities: 6-8 weeks per year required
